Payment is expected at the time of service. We accept cash (exact change only), checks, and most major credit cards. Please review the fee schedule for a list of current fees located in the Intake Forms.
If you are unable to keep your scheduled appointment, we require a 24-hour notice. In the event you do not give a 24-hour notice, you will be subject to a late cancellation fee. All fees must be paid before future appointments can be scheduled.
We are an out-patient practice. Our outpatient providers do not have admitting priviledges at local hospitals. The emergency phone numbers provided on the office voicemail are for after-hour urgent issues that are not life threatening. If you are experiencing a life-threatening emergency such as violent or suicidal thoughts, please call 911 immediately.
Prescription refill requests are approved during office hours only and require up to three (3) business days for processing. Medication samples and paper prescriptions must be picked up during office hours.
It is your responsibility to notify us of any insurance changes and to obtain any required authorizations. We will submit claims to the insurance company that we have on file. Per our contractual agreements with the various insurance companies, we must collect all copayments, coinsurances, and/or deductibles from you. Payment is due at the time of service. SHould the insurance company not cover the service, the balance may become payable by you.
Our practice is HIPAA-compliant which means that all communication with our staff is protected health information and cannot be released to any other party without your written consent. All clients or their parent/legal guardian must sign a release before any information can be released.
The items listed below are not covered by insurance companies and are your responsibility. Fees for these items must be paid at the time service is rendered.
