Thank you for your interest in Gulfcoast Behavioral Health.

We respectfully request that you complete our intake packet below and submit all required documentation prior to scheduling an appointment.   ​  

When sending these documents, please also include a picture of your your government-issued photo ID as well as the front and back of your insurance card. 

Completed packets can be emailed to:

Please do so only by email or regular U.S Mail. 

Fax copies of ID cards do not transmit well and we are unable to use them. 

DUE TO HIGH DEMAND Please allow us up to ten (10) business days to review your intake forms and verify your insurance benefits before a member of our staff calls you to schedule your evaluation.

Please be advised that our talk therapists are not accepting new clients at this time and our Psychiatric Nurse Practitioners that are accepting new clients are currently scheduling in mid-April 2025.
