Please submit documents by email or regular U.S. mail only. We are unable to accept faxed copies of identification or insurance cards, as the image quality is often insufficient for processing.

Our Psychiatric Nurse Practitioners are accepting new patients and are typically able to schedule appointments within 1 to 4 weeks, depending on provider availability and your preferred appointment day and time.
